Sunita, a medical student, is investigating the anterograde amnesia case of Sanjay Singhania, the chairman of Air Voice, a telecommunications company. She does so against her professor Dr. Debkumar Mitra’s wishes, in spite of Sanjay being under criminal investigation. Sanjay, who loses his memory every 15 minutes, uses a system of photographs, notes, and tattoos on his body to recover his memory and remember his mission of avenging the murder of his fiancé Kalpana Shetty, who was killed by a criminal kingpin, Ghajini Dharmatma.

Police officer Arjun Yadav, who is investigating a murder that Sanjay committed, knocks Sanjay unconscious in a scuffle at his apartment and finds two diaries in his drawer. He learns that Sanjay, a successful entrepreneur, met Kalpana, a struggling model, after he planned on installing an advertising billboard above her apartment. When his agents arrive at Kalpana’s shooting site to negotiate with her about it, Kalpana’s boss, advertising veteran Satveer Kohli, misinterprets this as a romantic advance and, in a view of fame and fortune, encourages Kalpana to accept the overture. Kalpana also decides to pose as Sanjay’s girlfriend. As soon as Sanjay finds out about the false rumor, he plans to meet Kalpana to confront her but fails, as he starts liking her after seeing her helping and kind attitude. He poses as Sachin, a newcomer model, and lies to Kalpana about himself. Soon, he falls deeper in love with her. At the end of the first diary, on 31st December, Sanjay, still playing Sachin, proposes to Kalpana who, taken aback by such a sudden gesture, asks for the night to think over it. He decides that if she would agree, then he would reveal his true identity to her, but, if not, then he would quietly walk away from her life as Sachin.

Ethan Mascarenhas (Hrithik Roshan) is a former magician who is a quadriplegic. He becomes the Radio Jockey of an FM Station called Radio Zindagi. His show spreads magic, hope, and laughter through his irrepressible wit and humor to every listener and caller, making it difficult to imagine that this is a man who has been immobilized with a spinal injury for the last fourteen years. Sofia D’Souza is his nurse who has been accompanying him for the past twelve years.

On the fourteenth anniversary of his accident, Ethan decides to file an appeal to the court for mercy killing. Ethan takes the help of his best friend and lawyer, Devyani Dutta (Shernaz Patel), to support his appeal. Devyani understands Ethan’s appeal and agrees to his reasoning and decides to support him in his cause. Surprising everyone with her stance, Ethan’s mother Isabel Mascarenhas (Nafisa Ali) also supports him in his petition, backing her son’s demand. Ethan’s physician Dr. Nayak (Suhel Seth) with whom he has consistently persuaded to retract his case, he finally relents to Ethan’s appeal when he realizes that the friend in him is much stronger than the medic. In the meantime, a young man named Omar Siddiqui (Aditya Roy Kapur) enters Ethan’s life to learn magic from him whom he considers to be the greatest of magicians. Impressed by his love for magic, Ethan agrees to pass on his legacy to Omar. Later, Omar confesses to Ethan that he is the son of Yassar Siddiqui, who happens to be responsible for causing Ethan’s accident. Ethan knew of this the entire time, yet that didn’t come in the way of passing his legacy to Omar.

On the day of verdict Ethan’s plea is rejected by the court, saying the legal code of the country cannot be violated in any circumstance. Ethan spends his time alone in his empty home, while Sofia was taken by her husband. When Sofia returns, she confesses to Ethan that she got the divorce and tells him that she will help with his euthanasia, whatever the consequences be as Ethan means more to her. On hearing her words, Ethan realizes how much Sofia loves him. He proposes to her and Sofia agrees.

Ethan throws a farewell party for their friends and guests, where Ethan speaks about everyone who has meant very dear to him and finally tells everyone about his and Sofia’s love. Ethan says that he will be dying a happy man with no regrets and a heart full of Sofia’s love, and bids all goodbye. At these words, all the guests embrace him, with Ethan laughing heartily.

A journalist reports on the life of Mahi Arora (Kareena Kapoor), a renowned actress and film heroine, describing her as unstable and problematic, but damaged and lonely due to childhood trauma, and bipolar disorder. Through a flashback, we are shown Mahi’s relationship with leading actor, Aryan . Aryan is going through a divorce with his wife whilst dating Mahi. The couple makes a sex tape of themselves. When Mahi comes to know about an intimate scene Aryan is shooting for a movie, she creates a scene on the set, which upsets Aryan. The couple reconciles but Mahi is repeatedly insulted by Aryan’s wife. While driving back from a party with Aryan, she raises the topic of his wife, which results in her being thrown out of the car.

Realising her place in Aryan’s life, Mahi goes into depression, putting her career in decline. A friend convinces Mahi to leave the past behind her and re-enter the industry so Mahi decides to rejuvenate her career. She employs a public relations manager, Pallavi (Divya Dutta), who uses sensationalist tactics to reinvent Mahi’s image. Mahi starts a relationship with the Vice Captain of the Indian National Cricket team, Angad Paul (Randeep Hooda). Soon, her personal and professional life is stable once again. On the advice of Pallavi, she uses Angad’s connections to get a role in a big budget film. The lead actor of the film, Abbas Khan, a married playboy, attempts to initiate an affair with her. She rejects him, which causes him to seek revenge. He demands that the director re-edit the movie so that story becomes about him and an item girl. Mahi is dissatisfied despite the film being a success, and in a desperate attempt to prove her acting skills, works in a low-budget art film for months. Angad proposes to her but she feels that they should concentrate on their careers. This causes their relationship to end. The art film never completes, and Mahi goes into depression again. She is also denied the opportunity to adopt a child, due to her history of alcoholism and psychiatric problems. Since she cannot get a role in big-budget productions, she decides to work on a low-budget film with newcomers.

At Rahul Arora’s (Samir Soni) fashion show, Meghna meets supermodel Shonali Gujral (Kangana Ranaut) and is ridiculed by the show’s choreographer, Christine, and told to have her portfolio photographed by Kartik (Rohit Roy). To afford Kartik’s fees, Meghna shoots a lingerie advertisement; she is mentored by another aspiring model, Janet (Mugdha Godse). Meghna’s lingerie photos appear on the cover of a magazine; her relatives in Mumbai see the cover and ask her to leave the house. She moves into an apartment with Maanav. Meghna is noticed by Anisha Roy (Kitu Gidwani), an executive at Panache, a major modeling agency. Anisha introduces her to her superior, Abhijit Sarin (Arbaaz Khan), who is impressed by Meghna’s ambition and confidence. Panache’s top model is Shonali, but her drug abuse becomes problematic. Abhijit includes Meghna in a fashion show organised by Vinay Khosla, but is replaced after a misunderstanding. Abhijit consoles her, and decides to replace Shonali with Meghna as Panache’s new top model.

Meghna becomes an overnight success, ends her relationship with Maanav and begins an affair with Abhijit. Meanwhile, Janet goes to work for designer Rahul Arora. Rahul’s mother becomes increasingly concerned about his sexual orientation so he asks Janet to marry him. Shonali’s drug abuse worsens and she attends a rehabilitation clinic. Meghna pays the price of her increasing fame by losing her close friends due to her changed attitude towards them. She becomes pregnant with Abhijit’s child, and reluctantly has an abortion due to conditions in her contract. After realizing that she was deceived in her relationship, Meghna tells Abhijit’s wife about their relationship, and Abhijit ends Meghna’s contract with Panache. Upset at the turn of events, Meghna descends into alcoholism; at a rave party she uses cocaine and unwittingly has a one-night stand. When sober, she feels guilty and returns to her parents in Chandigarh.

Ronny beats up Raghav’s men when they manhandle Subbu, who Ronny is close to, only for Raghav to get him arrested later on. Guruswamy tries to persuade Raghav, who turns out to be his son, to stop pursuing Sia, but Raghav poisons and kills him to get him out of the way. Ronny is heartbroken to hear of Guruswamy’s death since he had seen him as a father figure. Khurana then creates a misunderstanding between Sia and Ronny to separate them, which works and the two part ways.

In the present, Ronny reaches Bangkok and visits Raghav’s fight club, beating the most potent fighter there to get Raghav’s attention. The next day, Ronny breaks into the house of Raghav’s right-hand man, Biju, and threatens Biju’s wife at gunpoint, forcing him to reveal Sia’s location. Finding out she was at the hospital, Ronny saves Sia and escapes with her under both of them being disguised as a doctor and nurse. The two stop at an island on their way back to India, where Sia discovers her father’s deceit when she sees that Ronny is still wearing her ring. The couple reconciles, but Raghav and his men attack them. Biju shoots Ronny, who falls off a cliff.

Raghav takes Sia back to his place. It is then revealed that Ronny is alive. It turns out that Biju had used blank bullets in his gun, sparing him as Ronny had spared his wife’s life earlier. Raghav kills Biju when Biju says that he believes Ronny to be correct.

The film opens in 2016 BCE with Sarman (Hrithik Roshan), a young man from the village of Amri, who lost his parents at a young age. Sarman kills a crocodile that has been terrorizing his village’s fishermen and is hailed as a hero. He asks his uncle, Durjan (Nitish Bharadwaj), to allow him to go to Mohenjo Daro to trade their family’s goods, but his uncle refuses. Sarman attempts to sneak away to the city at night with his friend Hojo (Umang Vyas), but is caught by Durjan, who relents and allows both friends to go. He gives Sarman a seal that contains an inscription of a unicorn that Sarman often sees in his dreams, suggesting he use it only once in a life or death situation.

Arriving in Mohenjo Daro, Sarman learns that the city is ruled by the tyrannical Senate Chief Maham (Kabir Bedi) and his wicked son Moonja (Arunoday Singh). He also learns that the unicorn he sees in his dreams is the symbol of the city, and feels as if the city is oddly familiar to him. While Sarman is trading, Maham proposes to impose an additional tax on the farmers so that the city may grow, but Sarman leads the farmers to oppose the taxes so that their families don’t starve to death. Sarman gains access to the upper city by showing his uncle’s amulet and meets Chaani (Pooja Hegde), the elegant and gorgeous daughter of the head priest (Manish Choudhary) Mohenjo Daro. Sarman is enchanted by Chaani’s heavenly beauty and charm and falls in love with her. Upon meeting, the head priest strangely appears to recognize Sarman. Chaani reveals that she has been forcibly betrothed to Moonja, Sarman’s enemy, who is cruel and ruthless. Maham discovers Sarman and Chaani love each other and that Sarman is the leader of the tax revolt, and so he challenges Sarman to fight Bakar and Zokhar, his two champions. Sarman proposes that if he wins, Chaani will be released from her engagement, and Maham accepts the terms.

The head priest reveals to Sarman how Maham was expelled from Harappa for illegal trade with the Sumerians. Maham entered Mohenjo Daro as a trader and quickly rose to become the trade chief. Maham had discovered that the mighty Sindhu River held vast gold deposits, so he decided to place a dam on the river and divert its course to mine the gold. The wise Senate Chief Srujan (Sharad Kelkar), who is revealed to be Sarman’s father, opposed this, but Maham won the vote to build the dam. He had Srujan framed and arrested for hoarding gold. Chaani’s father and Durjan – Sarman’s uncle – were coerced by Maham to betray Srujan, and the latter was killed. Maham then took Srujan’s place as the new Senate Chief. It is now up to Sarman to defeat the evil Maham and avenge for his father.

In the arena outside the city, Sarman faces the ferocious Tajik mountain cannibals Bakar and Zokhar. After a vicious battle, he kills one of the cannibals but spares the other, and the people of Mohenjo Daro surge even stronger behind him. Enraged, Maham urges Moonja to finish off Chaani and the priest. Moonja kills the priest, but Sarman saves Chaani and kills Moonja.
